Office Policy

  • 24 to 48 hour notice is required before medication can be authorized for refill by your physician.
  • There is a $40.00 charge for completing any insurance or related forms. A period of 3-4 days is needed to completely process the forms.
  • Medical Records or X-rays may be released to you with the proper signed records release form completed. At least a 2 day advance notice is required for all records releases.
  • It is important to bring all insurance cards the day of your visit. All insurance information must be available such as subscribers name, DOB and SS#. If you do not bring your insurance cards, you will be considered a self pay patient. This will require you to pay at least a $250.00 deposit towards your bill prior to your visit.
  • It is your responsibility to inform us of any changes in your insurance since your last visit, such as address, a new group plan, a phone number.
  • Co-payments are due at the time of service. Durable medical equipment and supply costs are due at the time of receiving. Insurances do not cover this. (Examples: Cast, Sling, Cast Shoe, etc.)
  • Self Pay patients are expected to pay for services when rendered. A deposit of $250.00 is due at the time of check in and the balance to be paid at check out or a payment plan will be set up.
  • If you received an injury on the job (Workers Compensation) or an injury in an automobile accident, you must have appropriate authorization from insurance plans. You must notify the front office staff so that appropriate information is received from you. Auto accident victims must provide other health insurance information at time of service.
  • If your insurance requires an authorization from your primary physician, it is your responsibility to call them for the authorization, whether you are a new or established patient.
  • If we do not receive an authorization prior to your visit, we will need to reschedule your visit to a new day.

As with any orthopaedic practice, we are on call with the hospitals on any given day. This does require us to see emergency appointments at any given day or time. This does, unfortunately, backup our office hours and the wait time may be extensive. We will try to keep you informed of wait times at the time of check in and you are welcome to wait or reschedule your appointment. Please be patient with the staff. They are doing their best to keep you updated. Thank you for your patience.
